On the above vehicle we have a misfire on start up, engine has a miss a few times when moving off, all ok when engine is warm, have adjusted timing, new leads and plugs fitted, pcv valve replaced, ecu replaced 10 000 km's ago, air intake line ok , no holes, your thoughts.

Will have hydraulic lifters, try cleaning your throttle body, butterfly may have carbon build up around the edge, check for split hose on the intake side. Distributor cap and rotor may need a look at also. Timing or vac leak would be my choice.
